Who is responsible?

Which data, and why?

To record and confirm your appointment and remind you of it: your name, e-mail address and phone number. Without them you cannot book online (GDPR art. 6(1)(b)).

If you fill them in yourself: the reason for your visit and your national register number. The doctor uses them only for your care (GDPR art. 9(2)(h)).

From sales representatives: name, company, e-mail address and phone number, to arrange their appointment.

For a home visit the practice plans for you: also the address where it takes place.

For security: the IP address of a booking, a confirmation code by e-mail and the server logs. They keep abuse and outages out (legitimate interest, GDPR art. 6(1)(f)).

How long do we keep your data?

Your appointment: your contact details, the address of a home visit, the reason for visit, your national register number and the IP address are erased twelve months after the appointment. Only your name and the date stay in the practice's agenda.

Your patient file: so you do not have to enter everything again each time, we keep your name and contact details until five years after your last appointment.

Technical data: sent e-mails, logs and backups are erased after three months at most.

E-mails you send to the practice are erased once they are no longer needed; anything that matters for your care goes into your medical record.

The medical record is not on this website.

Who receives your data?

Everything is on a single OVH server in France, under a data processing agreement. The e-mails are sent from that server too. Your data does not leave the European Union and is not used for anything else.

Cookies

Only the cookies the site needs: your language choice, your request while you enter the code, and a confirmed e-mail address (thirty days). No advertising or tracking cookies and no analytics. No consent is therefore required.

Your rights

You can access your data, have it corrected or erased, have its processing restricted, object to it, or ask for a copy to take with you. You cancel an appointment yourself via the link in your confirmation e-mail; for anything else, e-mail info@hammersweg.be or call the practice.
